Company affiliation, MOS, place with 2/4 (Hawaii, VN, Lejeune, etc). Please enter the city and state you were from at the time of service and most important any NICKNAME you may be known by or used. You may also enter any persons you may be looking FOR. Indicate if this information may be distributed within the Association or if you want to be notified first.

After you click on Submit Form, you may be asked to return to the page...rest assured, your information was sent and will be entered into the log book. No need to re-fill the form although it may appear to be that way.
